DOH Activates ACF QSIR on HCS

(Jan. 3, 2026) The Department of Health (DOH) has activated the Quarterly Statistical Information Report (QSIR) on the Health Commerce System (HCS) for the final quarter of 2025 for adult care facilities (ACFs). All ACFs must complete the report no later than Jan. 31, 2026. Failure to submit may result in enforcement action, including the imposition of civil penalties.

Several HCS roles may enter QSIR data, including the ACF’s Administrator, Administrator-Backup, Health Provider Network (HPN) Coordinator, and Data Reporter. However, the Administrator or Administrator-Backup must review the QSIR data, complete the attestation statement, and submit the completed QSIR.
