Amsterdam Nursing Home and Mount Sinai St. Luke’s Hospital received the LeadingAge New York 2014 Innovation of the Year Award for their collaboration on the Smart E.D.
The goal of the Smart E.D. (Emergency Department) is to minimize the number of nursing home admissions by making E.D. resources available for diagnostic purposes. By enhanced communication, rapid diagnostic capabilities and an improved understanding of the services of the nursing home, the staff in the emergency department can often return the resident to the nursing home thus avoiding an inpatient hospital stay. As a result, inpatient hospitalization rates decreased significantly.
The award was presented by Daniel J. Heim, Executive Vice President of LeadingAge New York at Amsterdam Nursing Home on October 9, 2014. Representatives from both facilities, as well as members of the board of directors from Amsterdam Nursing Home attended.
