New York's submission to the federal government for a Fully-Integrated Dual Advantage (FIDA) demonstration program. The proposal would establish an integrated managed long term care program for dual eligible individuals in eight counties (NYC Boroughs, Long Island and Westchester) and develop managed fee-for-service health homes to serve high cost dual eligible individuals in the rest of the state.
