The Department of Health (DOH) has updated the application deadlines for the two outstanding Statewide Health Care Facility Transformation Program (SHCFTP) grants. Please note the following new dates:

Mother Cabrini Health Foundation
The Mother Cabrini Health Foundation has opened its 2024 grant application process. Letters of Inquiry (LOIs) are being accepted until Thurs., April 25, 2024. Applications are welcome from organizations that align with the Foundation’s mission to advance the health and well-being of vulnerable New Yorkers.
A note to current grantees: All requests for renewal funding of currently funded projects must be submitted through this LOI process.
Timeline:
- LOI submission window: April 1st – April 25, 2024
- Invitations for full proposals: June 2024
- Grant decision: October 2024 and December 2024

Brookdale Foundation
The Brookdale Foundation’s National Group Respite Program supports community-based efforts to develop and sustain social model group respite and early memory loss (EML) programs for people with Alzheimer’s disease and their families.
In the fall of 2024, the Foundation will be awarding seed grants for the development or expansion of dementia-specific Group Respite programs or specialized programs for people with EML. The grants are $20,000 for the first year, with an opportunity for a second-tier grant of $10,000. Any 501(c)(3) or equivalent not-for-profit service agency wishing to start up or expand a community-based day program for people with dementia, their families, and care partners is invited to apply. Proposals are due by Thurs., July 25, 2024 to the Brookdale Foundation Group by email.

Field Hall Foundation
Field Hall Foundation is accepting LOIs for its fall 2024 grant cycle. The deadline to submit is May 31st, and grantees will be notified of grant decisions at the end of September.
The Foundation supports programs and projects that directly improve the lives of low-income older adults and their caregivers. Priority is given to programs that address their most basic needs, including:
- Food insecurity
- Home-based care and aging-in-place services, including those that address social isolation
- Respite and support services for nonpaid caregivers
- Safety, security, and elder abuse
- Social work/case management, including emergency relief funding
- Transportation for older adults, caregivers, and home health aides
Funding requests can range from $5,000 to $50,000+; requests for small capital projects can be for up to $15,000.
