ADHC Telehealth Tool Kit

This tool kit is designed to assist and provide information to adult day health care (ADHC) programs in the delivery telehealth services. With the exception of DOH guidance, the forms and protocols contained within this tool kit are not endorsed by the State. The tool kit contents are recommendations and can be adapted to fit the needs or policies of individual programs. The tool kit will be updated as necessary.
