Person-Centered Care Planning
In response to federal HCBS Settings Rule requirements, the Department of Health has taken a new and serious interest in person-centered planning for recipients of Medicaid HCBS. To comply with the rule, ADHC providers must understand these new person-centered planning standards and develop a different approach to program operations, policies and procedures and care plans.
ADHCC is offering a half-day intensive training on Wed., September 11 at The Center at St. Camillus, Syracuse, on how to convert person-centered theory into practice. Attendees will learn critical aspects of documentation and how to develop plans with individualized interventions and goals.
Person-centered planning is not a buzz word, it’s a requirement for all ADHC providers.
