ADHC Person-Centered Care Planning Training, Syracuse

Person-Centered Care Planning

Registration is now open for Person-Centered Care and Care Planning in Adult Day Health Care to be held Wed., Sept. 11 at St Camillus in Syracuse. In response to member demand, this is the third session of person-centered training offered specifically for ADHC providers in 2019.

The training is designed for ADHC staff, including program directors, nurses, LPNs and social workers who are responsible for care planning. The training will cover HCBS Settings Rule requirements, critical aspects of documentation and policies and procedures to ensure compliance with the Rule. A significant portion of the day is dedicated to small group work where attendees will develop care plans with individualized interventions and goals.

The training has been submitted for 4.0 hours of continuing education credit for nursing home administrators (NAB).

This program offers 4.0 hours of continuing education for Licensed Master Social Workers and Licensed Clinical Social Workers.
