Developing a Responsive Transformation Grant Application

The recent announcements of the availability of funding under the Statewide Health Care Facility Transformation Program (SHCFTP) have generated a great deal of member interest. In light of the complexity of the application process, LeadingAge NY has arranged for Anna Rizzo, Managing Director of Senior Living, Long Term Care at FORVIS, to review the grant opportunities and share her insights. Ms. Rizzo has been involved in the submission of successful SHCFTP applications in prior rounds and is a frequent presenter at LeadingAge NY events. About Our Speaker

Anna Rizzo is a Managing Director at FORVIS and has more than 40 years of experience providing health care consulting services to a wide range of long-term care and senior living providers. She assists clients in preparing rate appeals and cost reports to third-party reimbursement agencies. She also has assisted health care facilities in obtaining additional Medicaid reimbursement via the appeal mechanism by preparing the requisite appeal document, including quantification of financial and documentation of various needs criteria. Ms. Rizzo has assisted clients in financial modeling and preparing Certificate of Need applications including right-sizing; establishment of new programs/services; and construction, expansion, and renovation of nursing facilities. She also has assisted with the preparation of various successful grant applications (Health Care Efficiency and Affordability Law (HEAL), Transformation, Capital Restructuring, Vital Access Provider (VAP), and Vital Access Provider Assurance Program (VAPAP)).

Transformation Grants: Background

The Department of Health (DOH) and the Dormitory Authority of the State of NY (DASNY) announced in January three opportunities for funding under the SHCFTP IV. A wide range of providers across the health care continuum, including nursing homes, home care agencies, hospice programs, and adult care/assisted living facilities, are eligible to apply for grants under the following requests for proposals:

  • SHCFTP IV and V for Health Information Technology (IT), Cybersecurity, and Telehealth Transformation: Supports technological and telehealth projects that facilitate health care transformation activities, focusing on cybersecurity, electronic health records, telehealth, and health management tools. A summary is here.
    • Question due date has passed.
    • Applications due: March 13, 2024.
  • SHCFTP IV: Supports capital projects and debt retirement and restructuring, working capital, or other non-capital projects directly related to a capital project that facilitate transformation, including redesigning and strengthening quality, pandemic response, or other innovative models of care, to increase access to care and ensure financial sustainability of health care providers.
    • Questions due: Feb. 20, 2024, and should be submitted to DOH here.
    • Applications due: March 26, 2024, 4 p.m.
  • SHCFTP IV for Residential and Community-based Alternatives to the Traditional Model of Nursing Home Care: Supports transformation, quality, and innovation of residential health care services, including, but not limited to, the conversion of a traditional nursing home into separate residential-style homes; improving health outcomes and quality of the patient experience; creating and contributing to a financially sustainable system of care; or preserving or expanding the ongoing availability of essential health care services to the people of the community.
    • Questions due: Feb. 14, 2024, and should be submitted to DOH here.
    • Applications due: April 9, 2024, 4 p.m.

Prospective applicants are cautioned that they must be prequalified in the NYS Grants Gateway or Statewide Financial System (SFS) to apply. This is a document-intensive process.
