Quarterly Home Care Worker Wage Parity Forms are Due

It is time to submit the quarterly Home Care Worker Wage Parity Law certification forms before June 1st  for Licensed Home Care Service Agencies (LHCSAs) and other entities that contract with Certified Home Health Agencies (CHHAs), Long Term Home Health Care Programs (LTHHCPs) and/or managed care organizations (MCOs) who operate in New York City, Long Island and Westchester.

For entities such as CHHAs, LTHHCPs and/or MCOs that contract with LHCSAs, it is required to obtain on a quarterly basis written certification from LHCSAs. The forms are on a prospective basis and the form due before June 1st is for the time period of June 1, 2016 through August 31, 2016. Please note Attachment A-4 is the designated form and it states for the time period of March 1, 2016. It is recommended to add after your signature the date that you signed the form and the prospective time period.

LHCSAs are reminded that this form is submitted to the MCOs, CHHAs and/or LTHHCPs, not to DOH.
