LHCSAs Pilot "Offsite" Surveillance Project

The Department of Health (DOH) has asked LeadingAge New York to share this announcement with our Licensed Home Care Services Agencies (LHCSAs) members:

DOH will be initiating a pilot project to perform off-site surveillance activities of selected Licensed Home Care Services Agencies (LHCSAs) in all regions of the State. DOH will be selecting the LHCSAs to be included in the Pilot Project. This is non-voluntary and required for those LHCSAs that are selected.  

The DOH Regional Office surveillance staff will conduct specific audits to determine compliance with requirements in the following areas: 

  • Clinical Records;  
  • Personnel Records;
  • Emergency Preparedness;
  • Health Commerce System;
  • Quality Assurance; and
  • Complaint Procedures.   

The pilot project will start Feb. 1st. The selected LHCSAs will be notified of the specific audit they have been selected for, required materials to be submitted, contact information for submitting materials, and time frames for submission. The selected LHCSAs will receive notification via letter through the Health Commerce System. The selected LHCSAs will be required to submit the requested materials via fax to the appropriate DOH Regional Office. It is anticipated that providers will be informed of the findings of the audits though issuance of statement of deficiencies reports.
