Reminder: ACFs Must Submit Information to OTDA

The Office of Temporary and Disability Assistance (OTDA) issued a letter asking Adult Care Facilities (ACFs) to share which residents in your facilities are receiving State Supplement Program (SSP) benefits. The SSP program wants to ensure that there is accurate data regarding your residents and their mailing addresses. Along with the letter is a form to submit the information, which consists of the resident's name, last four digits of their Social Security number and the residence/mailing address for each SSP recipient. They are requesting that the information be submitted to the SSP program (not the Department of Health) by Fri., June 5, 2015.
