DOH Revises Several Mandated Forms

The Department of Health issued a Dear Administrator Lettter (DAL) announcing the issuance of new, revised forms.  DOH has been going through the process of updating and improving old forms.  The revised forms are available on the Health Commerce System and will be posted on our ACF/AL DOH Forms page later this week.
