Reminder: ACF QSIR Due April 30th
The Department of Health (DOH) has activated the Quarterly Statistical Information Report (QSIR) for adult care facilities (ACFs). All ACF and assisted living providers must complete the QSIR by April 30th. Click here to view the Dear Administrator Letter (DAL). DOH also provides the following attachments to assist you in the completion of the QSIR; members are encouraged to review to ensure accurate completion of the report.
