DOH Announces 11 Regional Population Health Improvement Awards

The Department of Health (DOH) announced on Dec. 5, the award of $26.5 million to support regional efforts to improve population health through its Population Health Improvement Program (PHIP). The PHIP is a data-driven effort to convene stakeholders at the regional level for the purpose of  promoting the "Triple Aim" of better care, better population health and lower health care costs. Activities will be organized through 11 regions statewide, with a PHIP contractor in each region serving as the neutral convener that supports the analysis of data and the dissemination and implementation of strategies to promote better health outcomes and reduce disparities. Stakeholders include health care consumer and patient advocacy organizations; behavioral health advocacy organizations; disability rights organizations; health, behavioral health and disabilities service providers; rural health networks; insurers and other payers; local public health officials and other local officials; local human service agencies; the business community; unions; schools and institutions of higher education; local housing authorities; and local transportation authorities.

The PHIP awards will be paid out over two years to the following contractors:
