Nursing Home Transition and Diversion (NHTD) Medicaid Waiver
After receving rehabilitation services at a nursing home, Hannah was transitioned to living in an apartment complex where she receives home health aide services and assitance with paying her rent through a special housing subsidy program.
Overview: A 1915c federal home- and community-based service (HCBS) waiver, uses Medicaid funding to provide supports and services to older adults and diabled individuals in their communities, enabling them to transition from receiving nursing level care in an institutional setting or divert prevent institutionalization. NHTD Waiver services are considered when informal supports, local, State and federally funded services and Medicaid State Plan services are not sufficient to meet the welfare and health needs to maintain a person in the community. NHTD offers17 different waivered services, including congregate and home-delivered meals, assistive technology, respiratory therapy and home and community support services (personal care services).
DOH has partnered with NYS Homes and Community Renewal (HCR) to offer housing subsidies for NHTD participants. Through the NHTD Housing Subsidy Program, the Housing Trust Fund Corporation pays rental subsidies so eligible participants can live in decent, safe and sanitary housing.
Eligibility: All of the following criteria must be met in order to qualify for the NHTD Waiver. Eligibility is determined by the Regional Resource Department Center (RRDC)
- Be between age 18 and 64 with a physical disability, or age 65 and older upon application to the waiver
- Be a recipient of Medicaid coverage that supports community-based long-term care services (All Services except Nursing Facility Service, Community or Outpatient Coverage with Community-Based Long Term Care, Outpatient Coverage with no Nursing Facility Services)
- Be assessed to need a nursing home level of care
- Chooses to participate in the NHTD waiver
- Be able to identify the actual location and living arrangements for participant when receiving NHTD waiver services
- Must need at least one waiver service as identified in Initial Service Plan (ISP)
- Services agreed upon in the ISP must meet regional and statewide cost neutrality (the cost of providing services to all of the participants in that waiver in a region of the state are less than the cost if a comparable population was institutionalized in that region)
- Be capable of directing his/her Service Plan or has a legal guardian available to direct the participant's Service Plan
- Be able to live in the community where health and welfare can be maintained
