Medicare Cost Report Due Date Approaching

Members are reminded that Medicare cost reports are due five months after the end of the provider's fiscal year, which for most providers is the end of May. Providers for whom fee-for-service Medicare represents less than 10 percent of total care days may request a low utilization waiver as described below. The waiver allows providers to file a greatly simplified low utilization report.

Information is available on the National Government Services Cost Report website. The waiver request form is posted here. The cost report contact person at National Government Services is Christine Chamberlain, Christine.Chamberlain@wellpoint.com, 315-442-4039.

The criteria to qualify to file a low utilization cost report are:

  • Less than 10 percent Medicare utilization, or
  • Less than $200,000 Medicare Part A + B net reimbursement.

Items required to be submitted for a low utilization cost report:

  • Certification Page (Worksheet S), signed in ink by an officer or administrator
  • Applicable S-series worksheets
  • Request for Waiver of Electronic Filing of Medicare Cost Report
  • Balance sheet and income statement (these can be worksheets from the cost report (i.e., F-series worksheets for the 1728-94 cost report)
