DOH Announces Universal Settlement Signing Event Locations
Nursing homes are invited to attend one of the Universal Settlement signing events listed below to sign and submit their final settlement release documents. DOH intends to distribute the release document and facility specific attachments this week. While homes are not required to attend as long as they submit the signed documents by the established due date of Dec. 5th, DOH is encouraging attendance to ensure the process is completed fully and quickly. The dates, locations and hours are as follows:
New York City
November 23, 24, and 25 from 9 a.m. to 5 p.m. at 90 Church St, 14th Floor, Conference Room 4 A/B New York
(for homes in New York City, Long Island and the following counties: Orange, Putnam, Rockland, Westchester)
Rochester
November 30th from 8 a.m. to 5 p.m. at 335 East Main Street (DOH Offices), First Floor video conference room, Rochester
(for homes in the following counties: Allegany, Cattaraugus, Cayuga, Chautauqua, Chemung, Cortland, Erie, Genesee, Jefferson, Livingston, Monroe, Niagara, Onondaga, Ontario, Orleans, Oswego, Schuyler, Seneca, Steuben, Tioga, Tompkins, Wayne, Wyoming, Yates)
Albany
December 3rd from 8 a.m. to 5 p.m. at Empire State Concourse, Meeting Room 5, Albany
