DOH Announces Rental Subsidy and Transitional Housing Support Initiative

The Department of Health (DOH) is making available a grant opportunity for one statewide organization to facilitate rental subsidies and transitional housing supports for high-need Medicaid beneficiaries. The program is one of many being created through the department’s Medicaid Redesign Team (MRT).

The MRT Supportive Housing Olmstead Housing Subsidy Program will provide up to $10 million for two years, and will target current Medicaid recipients receiving nursing home level of care, or who have been homeless, and can live safely in the community. Priority will be given to Medicaid enrollees who are currently living in nursing homes.

DOH intends to make one award to a single organization that is capable of delivering services to the entire State. It is anticipated that 60 percent of Olmstead Housing Subsidy recipients will be in the Downstate Region, which includes New York City, Westchester and Long Island, and 40 percent in the rest of the state.

Proposals are due by Dec.1, 2015. Application materials are available through the Grants Gateway.
