Alzheimer’s Disease Caregiver Support Initiative

The Department of Health (DOH) announced last week the release of a Request for Application (RFA) for the Alzheimer's Disease Caregiver Support Initiative. This will provide support to caregivers of individuals with Alzheimer's Disease (AD) and other dementias. The total award amount is $15 million per year for five years. This could be one entity or ten regional entities across the State during the five year period.

The goal of this initiative is to promote early diagnosis of AD, delay the institutionalization of individuals with AD and maintain the best quality of life for individuals with AD and their caregivers.

The RFA outlines the core services identified within caregiver support services on page four:

  • fare consultation;
  • Family consultation;
  • support groups;
  • education and training programs; and
  • respite.

On pages 10-12 of the RFA, it outlines specific program expectations. Pages 22 -30 covers how to complete the application and how much weight will be given to sections of the application.

Specifics to note:

  • A Letter of Interest is not required, but it is strongly encouraged by this Fri., June 19.
  • Questions are due by Tues., June 23.
  • Questions, answers and updates will be posted around Tues., June 30.
  • Applications are due by 6 p.m., Tues., Aug. 4.
