Reminder: ACF Quarterly Statistical Reports Due Aug. 26

The Department of Health (DOH) activated the Adult Care Facility (ACF) Quarterly Statistical Report on the Health Commerce System (HCS). There are two reports that must be completed; the first report encompasses the time frame Jan. 1 through March 31, 2014, and second from April 1 through June 30, 2014. All ACFs are required to complete the report, which is due by Aug. 26, 2014.  

Additionally, facilities with a certified bed capacity of 80 beds or more, in which 20 percent or more of the resident population are persons with serious mental illness, are required to provide additional information. Facility-specific information has been sent directly to transitional adult homes, and more information is available in the report instructions.
