UHF Publishes New Report and Toolkit to Help Patients Transition from SNFs to Home

In February, United Hospital Fund (UHF) published a new report and toolkit providing resources to help skilled nursing facilities (SNFs) and other health care organizations ease the transition to home for both patients and their family caregivers. The report and toolkit grew out of UHF’s SNF Learning Collaborative, a two-year partnership with eight LeadingAge NY members working to improve care transitions: ArchCare at Ferncliff Nursing Home; ArchCare at Terence Cardinal Cooke Health Care Center; Gurwin Jewish Nursing & Rehabilitation Center; Jamaica Hospital Nursing Home, The Center for Comprehensive Nursing and Rehab; The New Jewish Home, Sarah Neuman; Parker Jewish Institute for Health Care and Rehabilitation; The Schulman and Schachne Institute for Nursing and Rehabilitation; and Sea View Hospital Rehabilitation Center & Home.

The report was written by Joan Guzik, director of quality and efficiency at the UHF Quality Institute, and Pooja Kothari, consultant at X4 Health and formerly with UHF. It was supported by the Mother Cabrini Health Foundation. Click here to access the report and toolkit resources.
