Healthcare Personnel Influenza Vaccination Report Due June 1st

The Healthcare Personnel Influenza Vaccination Report must be electronically submitted to the Department of Health (DOH) by June 1, 2021. All facilities covered under New York Codes, Rules, and Regulations, Title 10, Section 2.59 (10 NYCRR Section 2.59) are required to document the number and percentage of personnel vaccinated against influenza for the current season and to complete this report upon request of DOH. This includes nursing homes, hospice, and home care providers, including home care associated with assisted living programs (ALPs). Click here to view the Dear Administrator Letter (DAL).
