DOH Revises Chronological Admission and Discharge Register Form

On Oct. 14th, the Department of Health (DOH) issued a Dear Administrator Letter (DAL) to adult care facilities (ACFs) announcing changes to the Chronological Admission and Discharge Register.

These changes came about due to a law passed in the last legislative session requiring ACFs to collect data regarding residents’ races and ethnicities. The law specifies that this data be recorded in the Chronological Admission and Discharge Register. DOH has thus revised the form, DOH-5177, which can be viewed here, and will propose updated regulations to comport with the statutory change. DOH will likely add this data to the Quarterly Statistical Information Report (QSIR) as well.

LeadingAge NY had advocated for amendments to the statute to capture the data in a more logical place; however, the law was passed as written.
