State Announces Round III of Statewide Health Care Facility Transformation Program

On Sept. 30th, New York State announced the third round of funding available through the Statewide Health Care Facility Transformation Program. This funding opportunity will be of interest to many members rethinking or reconfiguring their services and facilities to maintain financial viability and improve access to long term care services. Please note the eligibility of several providers listed below. A total of up to $208,294,869 is available through the Request for Applications (RFA #18406) to health care providers in this round. The objective of the Statewide Health Care Facility Transformation Program III is to provide grants in support of capital projects, debt retirement, working capital, or other non-capital projects that facilitate health care transformation activities including, but not limited to, merger, consolidation, acquisition, or other activities intended to:

  1. create financially sustainable systems of care;
  2. preserve or expand essential health care services;
  3. modernize obsolete facility physical plants and infrastructure;
  4. foster participation in alternative payment arrangements including, but not limited to, contracts with managed care plans and accountable care organizations;
  5. for residential health care facilities, increase the quality of resident care or experience; or
  6. improve health information technology infrastructure, including telehealth, to strengthen the acute, post-acute, and long term care continuum.

Members should register now in the Grants Gateway to ensure that they can submit their applications in a timely manner. Registration in the Gateway requires some time to complete. We encourage members to engage in this step immediately.

Eligible provider types include:

  • General hospitals licensed under Article 28 of the Public Health Law (PHL);
  • Hospitals licensed under Article 28 of the PHL and designated as a Regional Perinatal Center under NYCRR 721;
  • Residential health care facilities licensed under Article 28 of the PHL;
  • Adult care facilities licensed under title two of Article 7 of the Social Services Law (SSL);
  • Assisted living programs approved by the Department of Health (DOH) pursuant to section 461-l of the SSL;
  • Children’s residential treatment facilities licensed pursuant to Article 31 of the Mental Hygiene Law (MHL); and
  • Community-based health care providers, which, in accordance with statute and for purposes of the RFA, are defined as:
    • Diagnostic and treatment centers certified or licensed under Article 28 of the PHL;
    • Mental health clinics certified or licensed under Article 31 of the MHL;
    • Alcohol and substance abuse treatment clinics certified or licensed under Article 32 of the MHL;
    • Clinics licensed or granted an operating certificate under Article 16 of the MHL;
    • Home care providers certified or licensed under Article 36 of the PHL;
    • Hospices licensed or granted an operating certificate under Article 40 of the PHL; and
    • Primary care providers with a New York State Medical License.

A minimum of $529,611 of the total amount must be awarded to community-based health care providers listed above. A minimum of $23,138,160 must be awarded to residential health care facilities, and up to $5,000,000 of the total awarded funds may be made available to Regional Perinatal Centers to establish telehealth applications.
