Amy Nelson, RN

Policy Analyst/ProCare Consultant

Amy Nelson is a registered nurse with 28 years of experience in healthcare, 16 years of which have been in long term care and 12 in Home Care.  Amy has held the positions of DON, ADON, Nurse Manger, Nursing Supervisor, Staff Education and Development, Employee Health, and Infection Control including Antibiotic Stewardship, and C.N.A.  She has been actively involved with policy and procedure development, quality assurance and regulatory compliance.

 

Carrie Mosley, BSN, RN

Policy Analyst/ProCare Consultant

Before joining LeadingAge NY, Carrie has worked in both the skilled nursing and Assisted Living settings. Carrie is a Registered Nurse with her Bachelor's Degree in Nursing. Carrie has 17 years experience in the healthcare field, all of them in long-term care. Carrie has held a variety of administrative and senior-level positions including, Unit Manager, Quality assurance Nurse, Director of Nursing Services, also taking an active role in education and staff development of employees. Carrie received her train-the-trainer designation to be able to teach the CNA training course. Carrie's background and experience within the CCRC setting along with her strong regulatory and clinical knowledge base will provide our members with valuable resources.

 

Karen Puglisi, BSN, MHA, LNHA, RAC-CT

ProCare Consultant

Karen Puglisi is a Registered Nurse with a Bachelor’s Degree in Nursing, a Master’s Degree in Healthcare Administration, she holds RAC-CT certification and is a licensed Nursing Home Administrator.  Karen has 42 years of experience in the healthcare field; 24 of them in long term care.  Karen has held a variety of administrative level positions, including positions in Case Management/Discharge Planning, MDS Coordination, Quality Assurance and Director of Nursing.  Karen has also takes an active role in the education and staff development of employees, including the role of adjunct instructor at a local community college teaching the CNA 100-hour certification course.  Her experience and background provide her with a strong regulatory and clinical knowledge base.

 

Phyllis Rosenberger, RN, MSN

ProCare Consultant

Phyllis Rosenberger is an experienced RN with 40 years of experience, concentrating in geriatrics. She is an experienced director of nursing with a strong regulatory and clinical background. A certified nurse educator, she is passionate about nursing education with her MS in nursing education, and has presented in numerous educational forums. She has worked with the School of Public in Albany in both GoldSTAMP and the epidemiology programs. Phyllis has an extensive interest in nursing informatics and conversion to electronic medical record, as well as training for nursing leadership. Phyllis also has extensive experience in training CNA's and helps facilities to administer training programs.

 

Gail Weinstein, MS, LNHA, CTRS

ProCare Consultant

Gail Weinstein, MS, LNHA, CTRS is a licensed nursing home administrator and certified therapeutic recreation specialist. For more than 30 years, Gail's long-term care experience has ranged from a hands-on recreation therapist to supervisory and administrative positions. As Vice President of Community Health Services, Gail was responsible for the creation, development, implementation and oversight of three Alzheimer's adult day health care programs, a long-term home health care program and an in-house day care program for nursing home residents with dementia. She is co-author of a chapter entitled "Approaches to Special Programming" published in Dementia: Issues in Nursing Home Care. Since February 2001 Gail has provided long-term care and adult day health care consulting services, through ProCare, conducting mock surveys, assisting facilities with POCs and DPOCs, and training staff in areas of accident prevention, documentation, behavior management, therapeutic programming, leadership/management skills and quality assurance.

 

Philip Mehl, LMSW, NHA

ProCare Consultant

Philip Mehl has worked in health care for over 20 years. As a Social Worker he has been a Director of Social Services Departments in urban and rural nursing homes. He also has experience working in acute care, medical surgical units, emergency departments and trauma centers. Philip did his undergraduate studies in Social Work at New York University and obtained his MSW from Columbia University. He received his Nursing Home Administrator's License in 2008 and oversees a facility in upstate New York that he brought to 5 Star status on the Medicare.gov website.  Philip brings to his work a strong focus on Resident Centered Care, staff culture, and sustaining quality initiatives through communication, mock surveys, audits, and root cause analysis. 

 

Elliott Frost, LMSW

ProCare Consultant

Before joining LeadingAge NY, Elliott Frost was employed by the New York State Department of Health as a senior social work consultant in the Division of Quality and Surveillance for Nursing Homes and ICF/MRs. In that capacity, he was responsible for numerous aspects of survey, complaint, enforcement and quality assurance activities. He provided guidance and assistance to providers, professional associations, advocacy groups and other state and federal agencies. Elliott also served as interim director of the statewide Centralized Complaint Program and director of the Criminal History Record Check Program.
