QIVAPP Reporting Deadline Extended

Please note that the Department of Health (DOH)  announced two extensions to the deadline on the Quality Incentive/Vital Access Provider Program (QIVAPP) reconciliation submission, with the most recent extension now updated to Fri., May 1, 2015.

The original deadline for the submission of the actual hours for each provider for the period April 1, 2014 through February 28, 2015 and projected hours for March 1, 2015 through March 31, 2015 and the accompanying attestations had been March 27.  

The most recent DOH deadline extenstion message:

In an effort to ensure sufficient time to gather and submit updated hours and attestations from the anticipated date and time of the informational webinar, the Department has now moved the required due date to Fri., May 1, 2015.

We will share webinar information once the date and time has been finalized.

Should you require any assistance or have any questions, please contact Department staff through the Managed Long Term Care Rate Setting email address (mltcrs@health.ny.gov). Please use the Subject line: QIVAPP Reconciliation.

DOH had previously announced that they are preparing to host the webinar referenced above in order to provide clarity on the submission instructions.

Original DOH message:

As described in the QIVAPP DAL dated April 23, 2014, the Department has the ability to adjust the QIVAPP award amounts based on subsequent reports of actual hours. Attached is a spreadsheet with your plan’s original list of providers submitted for QIVAPP funding. Please update the actual hours for each provider for the period April 1, 2014 through February 28, 2015 and projected hours for March 1, 2015 through March 31, 2015. These hours will be used to update and reconcile the full QIVAPP pool funding. Prior data submitted will NOT be used in final reconciliation.

Additionally, attached is an attestation to be used for each provider with updated hours submitted for reconciliation. Please attest and sign for each provider you are listing with updated hours. If it has been concluded that a provider does not meet the requirements, remove them from the list. If there is a provider omitted during the original submission, include this provider. This list, with attestations, will be used for final and complete pool reconciliation.

Please submit all documents to the bureau mail log MLTCRS@health.ny.gov by Friday, March 27, 2015 [old deadline].  No extensions will be allowed.
