Program Summary:
The Affordable Care Act requires the CMS Innovation Center to implement Health Homes under Medicaid, and Accountable Care Organizations (ACOs) under Medicare, in order to improve quality and reduce costs. Long term services and support providers have the expertise in care coordination and service delivery, and will play an important role in the implementation of these two new models of care.
During this audio conference we will define Health Homes and the different types of ACOs, how they are reimbursed, what services are provided, what types of providers are eligible to apply to be a health home and ACO, and existing best practices that relate to these models.
