Program Summary: The program will provide an overview of the NYS Adirondack Medical Home Pilot including its conception, goals and objectives, and progress to date. Information on specific programs put in place by the participating providers will be presented including programs to address transitioning of patients from one health care setting to another, disease management, care management and the empowerment of patients and their families to be active participants in managing their health. The program also will touch on current and future community collaborations opportunities.
